In case you have not heard, S. 1955, which would have had a potentially devastating effect on the profession of chiropractic, has been stopped in the senate! In the process of stopping it, some monumental organization occurred. A potentially devastating threat began an alignment process, and the profession went under acute care. A profession which for years has been in the state of chronic dis-ease of disorganization and dis-unification, unified and organized! Doctors from different backgrounds— straights, mixers, philosophical based, evidenced based, doctors from all techniques, doctors from all of the various schools—united! At least 20 or more State Associations participated in weekly teleconference calls to organize and effectively direct this acute unified force. National organizations, for the first time, had something on which they could agree and for which they could unify. The result was that a symptom went away—though none of us should think for a moment that an actual cure took place—as the adjustment was administered, but now the process of alignment needs to occur. Chiropractic must now continue in corrective care so that it may experience optimal health. Chiropractic, as a body, has been anything but in a state of optimal health. Our dis-organization and lack of unity has lead to many misalignments, with little or no organized feedback and regulation. Our schools are not booming and many of them are in real conflict; our regulatory boards have had scandals and have given the field reason for upset; our national organizations quibble; special interest groups and individuals within our profession have crossed over and aided the external environment to weaken us further. Yet, despite all of this, we are surviving—though perhaps less optimally than we can—and will continue to survive, if we live what the majority of us teach and demand that our leaders further the process of organization and unity so that we may begin corrective care. S. 1955 was a wake-up call and its best attribute was that it united us. We must now stay both united and organized in order to develop great internal body feedback and regulation, so that we can experience optimal health. We must, as a profession, continue to clean up—putting aside our individual interests for that which is for the greatest good of our profession as a whole, so that we are not always at the mercy of a hostile external environment. If you do not think the external environment is hostile, realize that Medicare has interpreted that care beyond twelve visits may be fraudulent, and the National Insurance Crime Bureau has estimated that $100 billion dollars each year is paid in fraudulent medical bills (http://www.statefarm.com/media/insback.htm). I can think of no better place to strengthen both our intra-professional feedback and regulation than the Council on Chiropractic Guidelines and Practice Parameters (CCGPP) “Best Practices Document,” a document not to be taken lightly as we all must live with it, and be judged by it. Most of us know that “Best Practice Documents,” like the earlier Mercy Document, can cause further dis-unity and dis-organization to our body. In perhaps a month or so, the first chapter will be released, “The Low Back Best Practices Draft.” This will be followed by drafts on the neck, extremities, etc. You must individually read this document, which can be downloaded at www.ccgpp.org. If you, like me, believe that this document does not fulfill its intent, then you must do something about it. It is not time to be emotional; it is time to be effective and our example is S. 1955. We must demand that our political leadership provide feedback and regulate what our body is allowed to put out as representing us as a whole. Do not sit back and think that this does not involve you, read the document and decide, and do it quickly. You matter and so does your opinion. If you do not like the CCGPP Low Back Document, immediately call your state and national political leaders and find out what they are going to do about it? Do not let emotion enter in; instead, organize! Let’s once again just be effective, and continue on our profession´s corrective care program—if not for ourselves, for the benefit of the millions of patients that rely on our care. Initial Draft of CCGPP Best Practice Document is Released The Council on Chiropractic Guidelines and Practice Parameters (CCGPP) has released the long-awaited initial draft of their Best Practice document. The culmination of more than two years of work, the draft is intended to provide clinicians, patients, payors, regulators and others with a tool to incorporate careful analysis of the current state of the scientific literature with clinical expertise and patient care options. We value input into the process from all stakeholders and hope you will assist us with getting word out that the document is available for comment. Production of this document has followed a carefully designed, scientifically based set of procedures intended to comply with the most rigorous and up-to-date international standards of best practice document development. Part of that process involves a carefully designed, pre-established search strategy of the scientific literature, with papers rated for scientific validity and utility. Areas of inconclusive or contradictory evidence were subjected to accepted consensus methodology, and the entire document is intended to be as transparent as possible, allowing users to determine the bases of recommendations and relevant source data for conclusions. Stakeholders, including chiropractic providers, patients, third party payers, students, and others, are being asked to provide feedback regarding the draft document through a survey mechanism on the CCGPP or Spine.com websites. The draft can be accessed at www.CCGPP.org, and lay users can access an abbreviated version at www.spine-health.com. Stakeholder organizations, including state associations, national associations, chiropractic colleges, and other interested organizations, are invited to provide specific comments, preferably by organizing a committee of interested members to review and comment on the document. U.S. House Passes Legislation to Expand Military Access to Chiropractic Measure calls for analysis of extending chiropractic care to reservists, retirees and dependents The U.S. House of Representatives recently approved pro-chiropractic language that would require the Department of Defense to ensure that chiropractic care is available to all members of the armed services through U.S. military treatment facilities worldwide. The language, backed by the American Chiropractic Association (ACA) and the Association of Chiropractic Colleges (ACC), was passed on May 11 as part of HR 5122, the National Defense Authorization Act for Fiscal Year 2007. “The passage of this language is a victory for our men and women in uniform, as they are one step closer to accessing the chiropractic care they need and deserve,” said ACA President Richard G. Brassard, DC. The bill, as passed by the U.S. House of Representatives, would require the Secretary of Defense to develop a plan for providing chiropractic health care services to all members of the uniformed services, as mandated by Public Law 106-398, National Defense Authorization Act for Fiscal Year 2001. In addition, HR 5122 would require the Pentagon to study the cost and feasibility of expanding the chiropractic benefit to military dependents, retirees and reservists. According to ACC President Frank Zolli, DC, “The progress that has been achieved to date with the inclusion of doctors of chiropractic in both the DoD and Veterans’ health care systems could not have been achieved without the grassroots assistance of chiropractic state associations, colleges, doctors and chiropractic students. Chiropractic Expansion Trip to Russia

The future of Chiropractic in Russia is getting closer with each trip. The September 2005 trip was a return to St. Petersburg to further coordinate the details of the first Chiropractic Clinic to be established in St. Petersburg, Russia. This clinic will be located in the outpatient clinic at the St. Elizabeth Hospital, one of the largest and most prominent hospitals in the northwest region of Russia. The delegates in attendance at this meeting included Sherry Durrett, D.C., Texas; Jill Howe D.C. and Mr. Larry Berg, Illinois; Alex Konovalov, D.C., Moldova; Lynne Auerbach, D.C., Texas; and Elena Kroutovskja, translator, representative and coordinator from the government of St. Petersburg. Our meeting began at the conference table in the office of the chief doctor with all the delegates and the appropriate hospital staff, including Dr. Boris Taits, Chief Doctor of St. Elizabeth Hospital, along with the hospital's Chief of Radiology, Chief of Physical Therapy, and Dr. Alexandre Taits, Director of the Outpatient Clinic. Also called into the meeting later was the Chief Financial Officer of the hospital. The meeting reviewed some of the details regarding the exact way that we could operate a separate Chiropractic Clinic there in the facility as well as the commitment by the hospital staff to the development of the Chiropractic program as part of the outpatient clinic offerings. The interest and support of Chiropractic on behalf of all members of the medical staff continues to be incredibly positive. The hospital will be providing ample space in their newly built outpatient clinic for the Chiropractic facility. Dr. Taits' level of commitment and excitement about bringing Chiropractic to his hospital is demonstrated by his attitude, willingness to provide initial financial support, and clinic space dedicated to the startup of this program. At the completion of the meeting, we toured the outpatient clinic where the Chiropractic Clinic will be located. We also toured the radiology department and the physical therapy areas of the hospital. During the tour of the PT department, we noticed some unusual equipment that we were not familiar with in the U.S., which included an ultraviolet light therapy that was used to treat ear, nose and throat infections. We observed a woman in the PT clinic getting a facial bone massage that looked completely relaxing and not typically any treatment you would see in a U.S. hospital that would be considered therapy. At the conclusion of the clinic tour, our excitement was high and we were all looking forward to the selection of the doctor that will be the first Chiropractor to practice in St. Petersburg. Dr. Alex Konovalov, 2004 Palmer grad, has been selected and after careful discussion with his family has accepted the opportunity. He is a native of Moldova, a former state in the USSR. He speaks Russian, has a great personality and ability to communicate, and has a passion to bring and help develop Chiropractic in Russia. He also understands that this is a once in a lifetime opportunity to be a part of making history and is very excited. After our five hour meeting, we departed the hospital and headed to a downtown cafe for a much needed lunch and recap of the direction and next steps needed to open the clinic. Following lunch, we then accompanied Dr. Jill Howe and Larry Berg to the train station for their trip to Moscow to finalize their adoption of seven year old Russian, Dimitri. They will return in one month to bring him back to the United States. The following two days were dedicated to sightseeing inside St. Petersburg and the countryside outside of town. Dr. Konovalov and the rest of the delegation had the opportunity to have a private tour of many the most important historical buildings by our guide and translator, Gennady Chentsov. We toured St. Isaac's Cathedral that was restored like so many of the buildings and palaces after WWII , the Hermitage, and the Peter and Paul Fortress with walls that were approximately 20 feet thick. We finished the evening by attending a performance of the Russian Ballet at the world renowned Mariisky Theater. The countryside included a tour of Peterhof, the summer palace of Peter the Great, with the magnificent gardens and gold leaf fountains that all culminated into the Gulf of Finland. It was Fall in St. Petersburg and the leaves were beautiful gold and red colors. One activity for the locals is collecting large Maple leafs to fashion a headdress that resembled a crown of leaves atop their head. We also toured St. Catherine's Palace. The private collections and paintings inside the Palaces were incredible and the expansive ballrooms with magnificent chandeliers and gold leaf moldings were simply elegant. All that was needed was a live orchestra with gentlemen in tuxedos and ladies in their evening gowns to complete the picture. The future of this project is very bright with the opening of the first Chiropractic Clinic in St. Petersburg in the coming months. The project also entails the establishment of a Chiropractic College or Institute Today Dr. Sherry Durrett, project coordinator, is working on recruiting doctors for the Chiropractic Faculty and other successful practitioners interested in practicing there on a short term (6-12 months) as well as longer term (12+ months) basis. Tom Spicer stared at the 9-year-old boy lying on his table. It was his first patient at La Clinica de Mariposa in Costa Rica and the only thing he could think was, “Oh, no.” Contorted with cerebral palsy and mentally retarded, Phillip was a far cry from the patients Dr. Spicer, a November 2004 chiropractic graduate from Northwestern Health Sciences University, had seen during his internships. Nothing in his training prepared him for the feeling of abject helplessness he felt as he looked at Phillip. There would be no gradual wading into chiropractic. No, Dr. Tom Spicer was thrust into his professional career — his doctorhood — by a boy who couldn’t even sit up. Phillip’s retardation combined with the language barrier created a difficult situation that had to be overcome before Dr. Spicer could begin working on joints that hadn’t moved since Phillip’s birth. He began where he could. He gently stroked the boy’s hair. He looked in his eyes and smiled softly. He murmured reassurances. The boy moaned and looked away, his fear evident. Dr. Spicer felt helpless. In frustration he thought, “This is not what I signed up for… this helpless boy writhing on my table while I don’t have a clue how to help him.” Eventually a trust was formed and Dr. Spicer was able to begin moving the boy’s thumb. Over two appointments spread out over two days, that was all he was able to accomplish — but for the first time in his life, Phillip could move his thumb. It wasn’t the miracle Dr. Spicer was hoping for when he decided to go to Costa Rica. But what he didn’t know when he planned his journey was that the miracles would not happen to his patients, but to him. From Intern to Doctor It began in September. Tom Davis, DC, a lecturer at Northwestern, brought four chiropractic students and a massage therapy student to Costa Rica for a month-long experience treating the poor at La Clinica de Mariposa. Dr. Spicer — then a T10 student at Northwestern — was one of those students. Before he left, Dr. Spicer imagined he would learn to be compassionate, to expect the unexpected, to broaden his diagnosis and treatment skills, and to learn to work in “less-than-perfect environmental conditions.” He journaled in the days before he left, pondering how he would be perceived and what he would encounter. But nothing prepared him for the reality of the La Clinica de Mariposa experience. Several times a year, Dr. Davis brings a handful of students from Northwestern to La Clinica de Mariposa, a traveling clinic that is part of a missionary organization. Doctors from across the globe journey to Costa Rica to provide health care to those in desperate need. The doctors trek from town to town, setting up primitive clinics in churches and schools. Although the doctors are housed in apartments in San Jose, Costa Rica’s capital city, the working conditions are rough. The weather is a study in extremes as temperatures reach the upper 90s and stunning downpours rattle the roofs. Biting insects — including tarantulas and ants — are a constant torment. And equipment is limited to adjusting tables and the doctors’ hands, making diagnosis reliant on touch alone. Dr. Spicer was prepared for the adversity — he knew the trip would be an adventure — but he wasn’t ready for the trusting gazes of his patients; gazes that told him they considered Tom Spicer an experienced doctor. Although he felt fairly confident in his skills, Dr. Spicer hit sharp reality when his first two patients were plopped on his table, their limbs twisted with cerebral palsy. Sweat beaded on his brow as he realized he had no answers. But as the steady stream of patients flowed in and out of the makeshift examining rooms, he went from uncertain intern to full-fledged doctor. He had no choice — many of the hundreds of patients who flock to La Clinica de Mariposa hadn’t seen a doctor in years, if ever. They were eager for attention and willing to trust the band of chiropractors from the North to ease their pains and help them regain their bodies’ normal functionality. Dr. Spicer developed a bond with many of his patients, including Michael, a teenager who worked in the fields of San Miguel. He suffered from knee and ankle pain that limited his ability to work. Although he was concerned that the American doctors couldn’t do anything to eliminate the pain, he approached Dr. Spicer. Dr. Spicer adjusted him and sent him home with instructions to come back the next day. Michael said he doubted he would return, but the next day, he was back grinning from ear to ear. For the first time in months he was free of the pain that crippled him. By his third visit to Dr. Spicer, the young man had opened his heart to him, offering the hospitality of his home should Dr. Spicer ever visit Costa Rica again. The scenario was repeated again and again over the four weeks Dr. Spicer spent in Costa Rica. Suffering, yet trusting, people came looking for healing — and found it from a man who hadn’t yet earned his doctorate. Witnessing his own healing abilities shocked and amazed Dr. Spicer. In the midst of oppressive heat, venomous insects, limiting language barriers and primitive working conditions, Dr. Spicer learned to trust the only thing he could — himself. “I now trust my hands and my heart and my head,” says Dr. Spicer. “I now trust how I feel. It’s nice to have X-rays, MRIs and advanced imaging, but for me, I’m now OK with just using my hands.” Today Dr. Tom Spicer is a different person from the man who traveled to Costa Rica in late September. His experience — which, in the best of circumstances, could be described as an adventure — taught him more about compassion, about diagnosis, and about himself than any number of classes or internships. “I began each day in Costa Rica nervous and scared. I had no idea what each day would bring — downpours, language barriers, conditions I had never seen. But by the end, I was able to tell myself that it’s going to be OK.
